A Case of Pityriasis Rubra Pilaris Successfully Treated with Ustekinumab

Abstract

|
Pityriasis rubra pilaris (PRP) is a rare, chronic cutaneous inflammatory disorders characterized by follicular hyperkeratotic papules, scaly erythematous plaques involving the face, trunk and extremities, that may progress to erythroderma. A 54-year-old man presented with 6 months history of generalized follicular hyperkeratotic papules, confluent scaly erythematous plaques with islands of sparing on whole body. Hair loss on the scalp and eyebrows was seen. The histopathologic findings were compatible with PRP. The patient was treated with systemic methotrexate, cyclosporine, acitretin and topical corticosteroid for about 2 years, the symptoms slightly improved. Subcutaneous injection of ustekinumab 45mg was started according to the treatment protocol for plaque psoriasis. After 5 injections, the patient showed remarkable improvement of symptoms and body surface area for skin lesions was less than 4%. Alopecia was also improved. Herein, we report a case of classic PRP successfully treated with ustkekinumab, which appears to be an effective and safe therapeutic option in PRP.
